APPLICATION FOR ADULT NAME CHANGE — official North Carolina AOC-SP-604, Rev. 12/25. The statewide application that opens an adult name-change special proceeding before the Clerk of Superior Court in the county where you live (G.S. 101-2 through 101-6).
Caption: the county, your current legal name, the name you are asking for, and the file number the clerk fills in.
Sworn contents: your birth facts and parents' names as shown on your birth record, county of residence, the requested new name, and your good and sufficient reason — with declarations covering the sex-offender-registry bar and the one-change Chapter 101 limit (G.S. 101-6).
