Who files and where: a parent or legal guardian files the statewide Name Change and/or Sex Designation Change Petition for an Individual Minor (UCS-NC2) in the county where the child lives — Supreme or County Court outside New York City ($210), or the NYC Civil Court ($65) or Supreme Court inside the city. For a name-only change, check "Name Change," complete Sections A-1, A-2, and C, and leave Section B blank. The petition discloses both the child (Section A-1) and the petitioning parent (Section A-2) — convictions, bankruptcies, judgments, and support obligations for each. Attach proof of the child's birth: for a New York-born child, a certified copy or certified transcript of the birth certificate (NYC births: city Department of Health and Mental Hygiene; elsewhere in the state: NYS Department of Health Vital Records); for a child born outside New York, a birth certificate, passport, or other legal document, translated if not in English.
